The Shift to Hospital-at-Home: Why it matters & how to be ready

November 11, 3:00pm, CST - 4:00pm, CST

Registration

Presentation Overview:

Hospital at Home programs that provide a combination of pre-hospitalization, acute, post acute and ambulatory care inside a patient’s home achieve better patient outcomes because patients heal faster in their own beds. Funding through CMS and many commercial payers has made establishing and providing Hospital-at-Home care delivery a strategic focus for health organizations. Yet many struggle to successfully operationalize this new level of care and prepare their teams for continued care delivery innovation. Join us to learn the following:
● Definition of a Hospital at Home program
● Why Hospital at Home matters to your organization today
● The incremental steps to deliver this new care model
● Key areas to address for Hospital at Home readiness
